Question about I-602?

Based on my experience, the form should be filled out by the doctor and sealed. It's not your responsbility. I understand that there is a section on the application that a physican has to sign but when Ins services did not request any medical examination they just requested for me to complete part C of the I-485 application and reason why i used a fradulent passport, and i completed both parts and now i want to know do i have to have a physican sign the second part of the application if they didnt request it. Another thing that im concerned about is that Ins services requested me to explain why i used a fake passport to enter this country but i entered this country with my real passport do i list this in my application or do i call Ins services to see what they can do
