Question about I-485

In I-485, Part 2, Application Type(check one)
"I am applying for an adjustment to permanent resident status because......"
There are 8 types from a to h. My case is concurrent filing with I-140 pending. However, it's not in any of the 8 types. Apparently the I-485 is the old copy before July 2002.

Which one should I choose?
 
Someone told me that you have to select (h) and write under that concurrent filing. Let me know if you heard the same.

I have the same question - using (h) writing "concurrent filing" does make sense. Any confirmation on this?
 
I wrote to a lawyer about this and he told me that, I should choose "a" as a direct applicant, and my spouse should choose "b" as a dependent applicant.
 
But they ask for a copy of the approved petition if you select (a) -- and an immigrant visa number -- what happens if you have I-140 pending?

I selected "a" and attached a copy of my I-140 receipt. Today I just got the receipts for I-485, EAD and AP.
 
concurrent

so even in concurrent filling, the I-140 package and I-485 package should be mailed seperated.
