Question about filling EAD

I have I-485 pending with NSC and currently I am working on EAD. last time in April 2004 I went oversea and came back on AP. I am appling for 2rd EAD.
The questions I have are :::::::::::question-14- from the application(manner of last entry) and question-15- from the application(current immigration status). What should I put for these questions. And also what is the current fee. thanks

What should I put for these questions.

*** send PASSPORT STYLE 2 Photographs ( Old style will not be accepted)
** also send Color copy of Passport page that has your Name, Photo, DOB on it AND Color copy of your Driving licence

Better send on 7th september becase If you send 2nd Sep it will reach 5 and 6 is closed, send on 7th so that is reached on 8 or 9 and pile of the incoming mail may be cleared in mail room
