Question about DS230 for my 2 year old son

Hi,

I have couple of questions about filling DS230 for my son.

My son (2 years old) will accompany my wife to US. They need to fill up DS230 form.
Q1. she need to fill seperate ds230 for my son?

Q2. If yes for Q1 then who will sign the form? my wife or thumb impression of my son?

Q3. As per my understanding, the answer to question 21a in part1 about people accompanying you will be N.A. ( because answer to question 14 and 19 will be N.A.). Please correct me if I am wrong.
