Should we just write the employer name on the first line
and then just fill out the names?

No, it's asking for employer details for all the applicants. So write the employer name and address in each of the 4 lines (or as many as are applicable in you case). If someone is not employed (or home-maker/housewife) etc., say so. For children, just write 'student'.
