Questiion 9 in DS-156

Thanks both for all the help and quick response. I have one more question regarding DS-156.

Question 9 asks for Other First and Middle Names Used. I have a peculiar situation. Though I have never used any other name, the father's name in one of childrens' birth certificate and their passport has the word 'bhai' appended to my name and in case of my other child has 'kumar' appended to my name. Also my wife uses my name as her middle name and consequently the mothers middle name in their birth certificate as well as the passport also has 'bhai' & 'kumar' at the end.

Now what do I answer in question 9 and what will my wife answer in question 9.
