Q.36 in DS156 help!

Hi,
I applied Labor through my previous employer.
Now I got H1B (7 th year)extension through new employer ,on the basis of LC pending with my prevouis employer.
Last month LC filed through new employer has been certified.Still I140/485 has not been filed by new employer.
Now for sending visa revalidation DS 156 form, what shouid I answer
for
Q. Has any one ever filed field an immigrant Visa petition on your behalf?

Previous LC is pending and new LC certified.

------LC is NOT immigrant Visa petition I-140 is Immigration Visa petition, your past or new employer has NOT filed I-140 till date for you and your answer has to be NO
Previous LC is pending and new LC certified.

passport question

I'm planning to bring my parents. i don't have copies of all the pages of the passport, i've only pages that are not blank. since i've sent my passport for revalidation i won't be able to send copies of blank pages. Is it enough if i submit copies that have visas and otherinfo.
