PWD form question

The employers representative thats handling my PERM case wrote my name as person requesting the form followed by my address. I already received the PWD( prevailing Wage determination) from State Dept of Labor and actually it was maled to my address.

Should she have put her own name as the person requesting the wage determination. SHould I have her reapply with corrections.

thats odd! usually employer gets the PWD. Check with some good lawyer. Because redoing is again another wait. Can you pls share how long did it take exactly to get the PWD for SWA? And which state.
