problem concerning filling my DS230 form

Hi to all
Pls i wish to ask if i am suppose to stapple my photo on any of my form i am supposed to return back to kcc; if yes , then which form? Is IT THE DSP_122 OR DS 230 PART 1 OR TWO
Or am i suppose to let it loose
Also do i have to write my name and case number behind the photo
Thanks alot for all your usual support and consideration

Don't use staple machine or glue, use paper clips to attach the photo to any of the two forms that is placed on top

Yes, it's important you write your name and case number at the back of the photo
