Please Help!

My sister as US Citizen applied for Green Card (I-130) for my parents and my other sister and brother. Recently, she received a 3-page letter from USCIS requesting additional evidence to proof relationship (although, she submitted everyones birth certificate). There are several examples in that letter explaining how that can be done. The only feasible one is sworn Affidavit by two individuals confirming the relationship (means my sister and my brother had the same mother, etc.). Did anybody here go through this similar process? Could anybody, please post typical format for this type of Affidavit? Does this Affidavit have to be executed in a court? Any thoughts are welcomed
 
I guess the answer all depends what was on the birth certificates that she sent in. IIRC they need to show both parents names, otherwise there is no way to prove the relationship. Some countries (e.g. UK) didn't used to give long-form birth certificates by default, so if you sent a short one, that may be the problem.
