Please help with some I-485 questions

Hi all. I recently had my I-140 approved, and I'm starting the AOS process. I'm a Canadian citizen, and I have some questions about the I-485 forms I hope someone can answer:

1) The instructions for form I-693 (medical exam) state that everyone must have a chest X-ray for TB. Is this correct? It seems excessive, when a skin test can be done.

2) What is the best way to provide proof of immunization. Will they accept faxed records, or records that I bring in personally, rather than having them sent directly to the doctor?

3) Part of the supporting material required for form I-485 is a copy of my birth certificate. Will a copy of my blue wallet-sized card suffice, or do I need the long form from the Canadian government.

4) Other supporting material require for form I-485 is an employment letter. Does this need to be a new letter, or can I send the one that was written for my I-140 EB-1 OR petition.

I have recently done my medicals as per I 485. So, I am answering those questions as per my knowledge and experience. Replies to the questions :

1. You have to take the skin test for T.B. that is, provided you do not have T.B. It is only if you have proved positive for the skin test that you have to take an X- ray: otherwise there is no need for the X-ray.

2.As for the immunization records, I do not know about the fax copies, but you can take your records with you to the doctor. No need for the physician to fax it to the doctor.

3. As for the birth certificate, I do not know whether the wallet size will suffice, but I do know that you have to give a copy of the original birth certificate.

4. As far as I know, it has to be a new employment letter.

You will need the long form of your birth certificate. You can request a copy from the Office of the Registrar General in your home (birth) province.
