Hi .... on the form DECLARATION OF RENUNCIATION OF CITIZENSHIP OF INDIA ON ACQUISITION OF CITIZENSHIP OF ANOTHER COUNTRY... i wanted to know what to fill in on the first question after I.....WHICH address do i have to fill in after my name ....do i fill in my current U.S address or do i fill in my place of birth, Tehsil, District, State and Country.....or i need to fill in both my current U.S. adrress and Indian place of birth with Tehsil, District, State and Country.... And second question I have/have not been married...here do i just circle one or write next to it married or not married..THANK YOU FOR YOUR REPLIES

Here is what I did...
Just filled my full name in the first blank after the "I ..............................................".
Place of birth.... I filled only the name of the city that is mentioned in my Indian passport....
Your full current address is required to be written/printed at the bottom of that form and need not be cramped into that file line of the form.
Based on feedback from others in a different thread, I decided not to confuse them with all the birth place environment... (Tehsil, District, State, Country, GPS coordinates et al...)
