Please help me Urgent - Name of Petitioner

Should i write my own details? like my full name and Address on the Name and address of Petitioner on DS230 Part 2?

Or write someone who is currently resides in US who knows me?

I also notice some from other samples:

I feel confused because i was told to write my OWN name and address on the Name and Address of Petitioner on DS230 Part 2, but i saw different way to write Name of Petitioner on above website.

Please advise me because i need to mail back the forms back to Kentucky Consular Center as soon as possible.

My current number:
2009AS0002xxxxx

Can i ask what's my chance to get the interview? when will be my interview? Besides when is the deadline to mail back the DS230 Part1&2, and DSP-122?

To those who are winners from 2005-2008, please let me know what did you write for Name and the address of Petitioner? Besides should i tick or cross the Yes/No box. I saw the sample on the internet they actually Cross it, which mean they put an x in the box of yes or no. Is this the correct way to fill up the form?

What is your relationship with the winner? You have to give more info for anyone to advice you on this issue.

Yes, you are the winner, cause you received the NL, but if you are married the principal applicant is the person that NL & CN came in his/her name, this is all meaning of the petitioner, if you don't have any spouse or children (Single Person), just fill your full name & address, in general the petitioner is the person who NL & CN in his/her name, avoid any other name & address, it's direct letter to the winner, and that is you or your spouse if your Online Entry was for married couple, I am not a lawyer but a DV2009 winner same like you, trying to help.
 
So, does a single winner have to put his name to 26-28 of the DS-230 form and then his name and address to 29 although 26-28 completely repeat 1-3?
