Please help me: Q#15 & Q#26 in OF-156 form

hi friends,
The OF-156 form have the following questions;
Q 15: Have you ever applied for a U.S immigrant visa ? yes/no

Q 26: Has anyone ever filed Immigrant visa petition on your behalf ? yes/no

Recently I filed for I-485.
Does this means do I need to say \'yes\' to above questions ?

Please help me. thanks.
 
Do you think "Immigration visa petition" & "Immigration visa" are same ?

It looks like above two questions are same.
Friends, what do you think ?

Is there any difference between "immigration visa" & "immigration visa petition" ?

Any help appreciated.
