please help J1 Clinical approval

I am currently holding J1 student/research visa. I finished my Ph.D last month. I applied J1 categorical change from J1 student/research to J1 clinical with surgery recidency offering letter from university. I called ECFMG several times, All they told me is that still under review at the deparment of state. Since I have a deadline ( July 1), do you know any other case like mine or what do you suggest at this point.What do you feel about getting approval. Thank you.
