Please help in filling DS 156/157 forms

I am sending my passport for H1 Re-validation and am filling forms DS-156 and DS-157. What should I write for the following questions


DS-156:

Question 25

Name and Telephone numbers of Person in US. Who you will be staying with or Visiting for Tourisim or Business?

Do i need to write my employer details or mine ?




DS-157 :

Question-8
--------------
Full Name and Address of Contact Persion or Organization in the United States (Include telephone Number)?
What should I write here? My name and address or my company name and address ?


Question 18
---------------
Have you made specific travel arrangements ? yes / no

I guess i have to write "no" as i am already in US. Should i write "presently in US" in the space provided below ?
