Please Help!! H4 revalidation in St.Louis

I switched company and got new H1, I also got my H1b stamped in my passport. For H4 visa to be stamped in spouse passport what are all the documents I need to send? Should the cover letter be from my wife or from me?
Also, I'm planning to send H4 application to St. Louis before July 16th. Please help.

-- you get new H1 approval Notice and you got H1 Visa stamp in passport, dont use H1 for h1 approval notice and for H1Visa stamp
For H4 visa to be stamped in spouse passport what are all the documents I need to send?

From my new company they haven't applied for my spouse H4, attorney mentioned it is not needed as her previous approval is valid till 2005. In this case should I just send her previous approval(from my past company). Should I also send my recent pay stub?
