Please Help--EAD Renewal

I am a Vermont 1 year I-485 case.(RD 09/06/02 still pending) I am not sure when I get approval.
I am applying for EAD Renewal of my own.
I have few question.

1.Though I have A# on my previous EAD, as my I-485 is not approved,
Do I need to mention A# or I-94 number on EAD renewal application?

2.I am working on EAD, so what should I mention my current status ?
Is it called 'ADJUSTEE' or 'PENDING ADJUSTMENT APPLICAT' ?

3.Do I need to enclose I-485 Receipt notice also or just previous EAD copy is enough ?

You guys are great and I appreciate any info you give in this regard.

2. You can use either
3. Both i485 receipt and copy of old EAD (or the document in which you received the EAD)
