Hi, everyone:

I have been following this forum and was helped tremendously in my understanding of immigration process. Now, I would like to ask your opinion about my change.
1. MD from foreign country
2. MS (master in medical science) in one of the most prestigious universities in the US
3. 15 publications, including 3 first author papers
4. Citation ~60
5. Currently in residency program with a job offer from community hospital with university appointment
6. Letter of recommendation from 8 people. 4 people from who I know, all from prestigious places, and 4 from independent refrees, who cited my publications and graciously agreed to give me a letter
7. One small grant within the department (although it belongs to a very prestigious university)

I heard all the horror stories and am a bit scared. Although I heard success stories with not so impressive accomplishments. I don't have any prestigious membership, or a big grants. I never reivewed a paper for a journal. I am not really confident and would like to ask your opinion about my chances. If you can give me any advice, it would be greatly appreciated!
 
It is doable with a good attorney.
You may need few more letters (10-12 will be ideal for your case).
If the papers are in good journals try to include the impact factor too.
University name does not matter for the IO.
Departmental grants can be added as an extra information.
3 first author is not bad.
60 citations are very good.
which category you want to go? EB1-OR will be worth if your post is permanent and univ can give a letter of support.
Little tough for EB1-EA.
NIW is another possibility.
 
NIW looks promising. For EB1-OR PhD is almost a must, so that might be difficult to get through. But you have an MD, so its probably worth a shot. Try both, the letters and the material is almost the same. But as NIW_help said, get a good lawyer.

Since you are in residency program EB1 OR is out of question. You need to file NIW or EB1A. But since most of the work is same file for both especially if you are from retrogressed country.
