Please advice on I-94

I have been in US on H1B and my wife on H4 since Jan 2007 and we had our I-94 valid till 30 Sept 2009 which is as per the Visa Expiry Date.
In Nov 2007, I transferred my H1B to another company and recieved a new I-94 vaild till 20 Nov 2010. Please advice me on the following question:

My wife has been to India for vacation in Sept 2008 and returned to US in Nov 2008. While entering into the US, the airport officer has given the I-94 till 30 Sept 2009 as per the visa expiry date.

Inorder to get the date back on I-94 as per the date on I-797A as 20 Nov 2010 where should I contact? Please advice on this..

----------------------- officer gave her correct I-94 if she only had Passport visa stamp till 30Sep 2009. If she has H4 Visa stamp till 30Sep 2009 AND your H1 transafer approval that is valid till 2010 then she was suposed to show valid Passport visa stamp AND your H1 approval notice to get I-94 valid till 20 Nov 2010.
Inorder to get the date back on I-94 as per the date on I-797A as 20 Nov 2010 where should I contact? Please advice on this..

--------------------She will file H4 extension with USCIS with form I-539and USCIS fee BEFORE her I-94 gets expired.(one can file extension 6 months advance of I-94 expiry date)
