Physician statement for J1 Waiver application

Hello,

I'm wondering what should be in the personal statment that DOS want with the J1 waiver application. DOS web site states that they're expecting a "Physician statement".

I would appreciate, if someone post their statements which they submitted with their cases. Obviously, xxxxxxing out personal data etc..
