Pay fees to USDHS or INS or BCIS?

I am in the process of renewing EAD and AP.

Latest EAD form mentions to pay the fees to INS.

Latest AP form which expired on 06/30/04 mentions to pay fees to USDHS. Is there new form somewhere?

By mistake I have one money order for both applications in the name of BCIS. What is the process to change name on the money order. Should I have separate checks for AP and EAD

INS, CIS, BCIS, USCIS, USDHS-INS, USDHS-BCIS, USDHS-USCIS... anything will do.
My theory... give money to uncle sam... they will open an account to encash it.
 
For AP: pay to "Department of Homeland Security"
For EAD: pay to "U.S. Citizenship and Immigration Services"

Do we need to submit a letter along with application?
