I just went through similar situation yesterday and this is what it came down to:

1) The name on your birth certificate is your legal name (as recognised in your home country) and as such, that is what you should write down there.
2) The name on your GC is of course what you should write in the GC column.

You should take your birth certificate along with your passport, SSN and of course GC to the interview. The officer will write the name on your birth certificate as your legal name and then he would ask you what name you want to keep. You will need to tell them that you want a name change where they would reconcile all of these names into whatever name you want.

I had to do this yesterday and it was what took about 15 of the 25 minutes that I was with the officer. There should be nothing to worry about though; just know what name you want to keep so that you can make it easy when you get there. I did not have my birth certificate and it took some time for her to find it in my file...so take yours when you are called for interview.
