It says every place you lived for the last five years and then the table says Dates (mm/dd/yyyy). My question

[1] Can I write month and year only:

[a] XXXX LANE, 6/1996 to 10/2002

xxxxx street 10/2002 to present.

or I have to find out the exact dd also?

I would say: try to find exact dates if you possible can, if not try approximate dates in mm/dd/yyyy format if not than give month and year only.
Go back to your airline tickets, bills etc. for posible help in locating your dates.

I wrote only months and years and it wasn't any problem
