Parent Name in Birth Certificate

Hi,

can someone please help me in this question, I got I485 RFE for my wife and they need birth certificate with both father/mother name . We have original birth certificate with Father name only so what should be do ? Should we submit affidavit from mother & father side including both the name ?

Get affidavits from your parents (one from father and another from mother).

---- And on affidavits parents full name should be written and Date of birth should be same as on her passport, place of her birth should be mentined as she has on her passport or what she Fillied up on form G325A. if she changed her last name ( surname after marriage) better also it should be mentioned that her name before marriage wasXXXXXXX and after marriage with YYYY she changed her name to>>>>>>>>>
you can find sample affidavits on www.immihelp.com and add the above to that Sample.
parenets should get Affidevits on 10 rupees stamp paper(India) and should be Notrized or signed by exective Magistrate(tehsildar)
