Overstay question (long story)

Couple of questions for I-765:

I am applying for:

"Permission to accept employment", since I've never had an EAD before, or "Renewal of my permission to accept employment", since I used to hold an R-1 visa that allowed me to work at a religious institution?

Question #10: Do I put down the expired I-94 number?
Question #11: I assume I answer "Yes" and put down the embassy information for when I received my original visa, and then include a copy of the visa as documentation?
Question #15: Do I put my Current Immigration Status as "Out of Status" or "Overstay"?

Permission to accept, since this is your first AOS and first EAD.

It will take a couple weeks to go to the 2 medical appointments for the I-693 and receive the results.

About the physical, where does it say I have to go for 2 appointments? I'm going in this afternoon to do the paperwork and draw blood, and then going in Monday morning to meet with the doctor. I should be clear other than the fact that I'll need the vaccinations, since I don't have a written record of those on me.
 
You will have to go back for the results and them to check your arm if the TB test came back negative.

Had blood drawn and skin test today. Told to come back Monday to meet with the doctor and check the skin test reaction. Bloodwork results will come in later in the week with the holiday. They told me that I should be able to do the rest by phone and they can mail me my results and the completed I-693.

Also scheduled an appointment with an immigration attorney for next Thursday.

I-485, G-325A (both wife's and mine), I-765, and I-130s filled out to the best of our ability with a couple of questions remaining about one or two of the questions. I guess I've done all I can for now, though it doesn't really put my mind at ease.
 
I did end up going with an attorney. Yes, it's a couple thousand dollars that I could've used, but I just wanted to be sure all my bases were covered without finding conflicting information.

My I-485/130/765 packet was received September 30th.
Had a fingerprinting appointment scheduled for October 28th, but my attorney helped me get it done the first week of October.
Received notice of appointment on December 2nd.
Received my EAD in the mail on December 17th.
Appointment yesterday, January 9th. Approved for Green Card.
 
Thanks for the update. Did they accept your I-864W based on your 10 years of Social Security contributions, or did you end up having to file I-864 and get a joint sponsor?
