OPT to H1B

Hi Gurus,

I have my OPT till the end of Dec 2004 and presently I am working.

I have few questions

1) What is the best time to apply for the H1B premium processing?

2) If I apply for my H1B and if approved, can I continue to work on my EAD.

3) what if my H1 is approved and I dont accept it and continue to be on my EAD? Can I do this...

Please help me in this regards.
