OCI application from India- surrender certificate

I become citizen and got my naturalization on Nov 18th 2010. After I received my US passport I have applied for renunciation and received SC and cancelled stamp on indian passport on Dec 3rd 2010.

Using same SC I have applied for indian entry visa and received it on Dec 4th 2010.

My question is :

Since I am visiting India for 5-5.5 months, I am planning to apply for OCI from India.

Do I need SC to apply for OCI application from India or cancelled Indian passport will work?
I forgot to make copy of SC and submitted to Consulate of Houston to get Indian entry visa.

Please advise me

-----------------They know that if person got Entry visa then he got SC and Indian passport got canceled stamp from consulate and then got Visa
