OCI app questions

Hi Guys,

Had a few questions regarding the OCI app.. will be applying to SFO.

1) Do they need the original Surrender certificate or just a copy of it?
2) In the List of docs it says "Printed Checklist for each applicant" .... Where do I find this?
3) Only 2 photos are needed right? (1 pasted on each Part A form). or do they need additional photos to be submitted in an envelope?

The list I have is

Part A (2 copies ) for each applicant
Part B ( 2 copies)
2 photos for each applicant pasted on Part A
Original US passport
Original Indian Passport (most recent)
Original Indian Passport ( which was used to enter the US the first time) - since I do not have color copies of Green Card
Copy of US Naturalization Certificate
Copy of Driver license (For address proof)
Fee 275$ per application
Self addressed trackable envelope ( 1 )
Surrender Certificate ( is it copy or original?)
copies of 1st 2 and last 2 pages of US passport
copies of 1st 2 and last 2 pages of Indian passport (which was used to enter the US the first time)

All photocopies to be self attested.

Is this checklist correct?

Got the answers to the 3 questions above. CGISF replied on all 3 of them

1) Can I send one cashiers check of $550 for mine and spouse's application?

Yes you can include one cashiers check for the entire OCI fee.
