Not received the plastic card

Me and my wife's I485 got approved in 04/12/2002 and we got the stamping on our passport in May 2002. We did not received our green card (plastic card) yet.
We went to the INS office in Texas during April 2003 do inquire about the status of the plastic card and get the stamping as well. The INS officer told us that still they are processing the green card and he simply stamped the passport for another year.

I have the following questions:
1) What could be the reason for taking so much time to get the plastic card?
2) Is there a way to expedite it? If so how?
3) Is it safe to travel to India using the stamping in the passport and without the plastic card?

Maharani - Have you called the 800 number? What did they say? Try being a little aggressive and contact your senator and congressman. Normally people get the card within 2 months. Did you change your address by any chance? I believe you could also apply for a new card (if lost/stolen). Good luck.
