Nonimmigrant Visa # for I-485; Expired H1B

Hi I am a U.S. citizen and filing for my spouse who is on H1B. I am filing for I-485 and need help in part II. Non-Immigrant Visa #

She was on a H1B that was issued in 2006. The visa was stamped on her passport when she made a return trip to India on that H1B. Then in 2009 she got a new job and a new H1B I-94 form. The new form does not have a visa stamped on it because she has not yet gone to India on the new I-94. So do I use the VISA# from the original H1B which is expired or a different one?

For consulate where visa was issued what do i use? do I use just country or city and country?

Any help will be great. Thanks.

-------------------use the visa number from her expired Visa stamp. attach her latest H1 approval copy . attach Copy of her latest I-94 that is attached with her H1 approval notoce. attach Front and back of latest I-94.
For consulate where visa was issued what do i use? do I use just country or city and country?
----------------- if her Visa was issued by Mumbai US Consulate then one need to write: Mumbai, India( Mumbai is City and India is country.)

I am also on the same boat. Did you have any issues putting in the old visa number in the passprt? What about the I-94? Should that be from the new aproved form?

------How one can write the Old visa number in passport? Visa nuber is on Visa stamp.

----------- Copy of Latest I-94 (front and back). FYI one should keep copy of each H1 approval and I-94(front and back) for future records.
