No Birth Certificate/Marriage Certificate for Mother

Hi,
Mom is in late 70s and She was born in Gujarat. I do not have Birth certificate nor Marriage Certificate.

I understand I need to get affidevit.

Can anyone please send me sample affidevit or link to sample affidevit for Birth and Marriage to be done by relative.
