NIW. What evidence to submit?

Hi everyone. This forum has become more useful than most of the immigration attorneys who usually give vague and rushed answers.
My question is: For someone who is opening his or her practice in MUA. The NIW process requires that evidence should be submitted that the practice is in a MUA and the practice is Internal Medicine (for my case)? What specifically should the evidence be( does having the Board of internal medicine itself is enough? plus for example liability insurance in Internal Medicine) What else? Any coments appreciated

You should contact your State Dep't of health for their requirements. You would need to get their support letter for your proposed practice. Unfortunately some states make their own rules on this and will not give you a letter unless you would be employed. Without their letter you have no physician in hpsa/mua NIW case.
You would need to come up with concrete evidence of your plan to open a practice, ie a practice plan, with projected cash flow etc, maybe a bank line of credit, your proposed practice site etc. Of course you would need all the usual stuff like medical licence, malpractice insurance etc.
Basically any evidence you could demonstrate showing you have taken or are taking concrete steps toward actually opening the practice would be useful. If the hospital is helping you set up, then a letter from them expaining that would also be useful.

Thank you very much. Do you know if Florida State Department of Health can issue such a letter or not? Anyone had any past experience with them?

Any state Dep't of health can do this. Why not look up the contact person for their conrad state 30 program and contact him/her. Often it is the same person, or atleast they will know who to refer you to. Generally speaking with that person will give you the best feel for this issue, where you are.
 
Hi State Department of health is very friendly.Contact Karen Lundburg she will guide you.What part of florida are u opening practice in?
 
This is what worked:

- commercial lease for office space
- listing of the insurance plan billing#s
- certificate of coverage from malpractice insurer
- copy of hospital priviledges at local hospital
- bank documentation of capital for 6mo operating expenses
- fluffy sounding business plan mapping out competitors and potential referrers

(never actually opened the office. on a second look the numbers just didn't add up in that community ;-))
