NIW for underserved area

What other dcouments are needed with I-140 for NIW for underserved areas. A few are mentioned in the INS site. specifically, is ETA 750B form needed? (this is needed for other NIWs) TIA

U need to file I-140,a letter from IGA granting waiver of J-1, 5 year contract , a letter from the employer that u r still working thats all u need i filled it in 2001 and got approved in 2002 hope this helps

My situation is slightly peculiar in that, I haven't started doing my waiver job yet. Just got the waiver approval from INS, employer is working on H1. I have filed I-140 with the evidence you mentioned and I-485 ,I-765,I-131 etc in one package. Somewhat unchartered waters here, but will have to wait and see
