Once upon a time such a thing was possible in some states eg Louisiana. In recent years eligibilility rules have been tightened up.
I am aware of someone who had infact done his then 3yr NIW committment by doind ER shifts in various ERs without holding down a specific 8-5 job.
Nowadays, most health dep't are looking for a defined job description.
Sadly much of these newly imposed restrictions etc were the result of past indiscretion on the part of users of these programs. It makes no sense to try to be smarter than these programs. They were implemented for a reason and the State Dep'ts of health are charged with the responsibility to oversee them.
Talk with them.
