NIW 5 years done, please see my documents listed, tell me if I need more to submit

NIW 5 years done, final submission? what papers do I need other than what I mentioned

please tell me if I need state 20 letter(stating that I have completed 5 years) is a necessacity and I am not in good terms with my employer but he has given me every year one letter stating that I have been involved in HPSA for first 3 yrs.

please read below and advise if I need anything more and thanks and thanking you very much for your patience

I do have following information about HPSA as a Proof of work :
1. contract of office rent provided by Comminsioner of town
2.Local (HPSA)Newspaper ad : when we opened clinic first time (news paper photo of ribbon cutting, with town commissioner present)
3. telephone bills/ heating bills / rental bills of HPSA office
4. Site official visit by State 20; and their letter of recommendation for NIW
5. Office visit by Americaid: and letter of site visit "in good standing"
6. letters from my sponsor for each year :2000/2001/2002/2003 saying I am actively involved in HPSA
7. list of patients who were under sliding scale of payment schedule: whihc was shown to State 20, after which they gave recommendation for NIW.
8. sign In and Sign Out sheets for each month and all years, indicating my days
vacation and sick days other holidays etc.,
9. copy of two letters from Health Department recognising my work and recommending me to see EPSDT patients(EPSDT certification)
10. list of patients who received vaccinations, in HPSA area
11. periodic advertisement in local newspaper and magazine locally(latest a month ago)
12. a letter from owner of property stating my rent
13. copies of insurance billing of patients
14. Malpractice copy
15. ECFMG certficate, USMLE step1,2,3 , state License to practice
Board Certification, USPTO patent certificate, Residency completion certificate
16. first 3 years W2's from sponsor, remaining 2 years : solo practice contract and schedule C and few months ago creatEd my own corporation and have W2 for last few months.
PLEASE TELL ME IF i NEED TO ADD ANTHING MORE

Knock them out !

Write a cover letter explaining that you worked for practice xx from yy to zz and that on x/x/yyy you opened your own office in the same location. Tell them that you ran it as single proprietorship from x to y and as corp from z to now. Tell them that you see patients irrespective of ability to pay and that you see x% medicare y% medicaid in your practice.
Basically explain the flood of documents you are about to unleash on them.


Do you have a 'policy handbook' ? If yes, copy the page stating that patients will be seen regardless of their ability to pay, race, blablablba.

Do you have staff privileges at your local hospital ? Have the chief of staff or your division head give you an affidavit that you are on active staff and providing full time services at your office location how you are a valued member of the local medical community etc.

You seem to be on good terms with the town commissioner. Have him write a letter that you are taking care of the townsfolks for 5 years by now.
