Dear Friends,
I practise in pediatrics primary care in health professionals shortage area(HPSA).My 140 is approved recently, based on PERM. I am always on H1B, never was on J1. You think ther is any chance for me to apply for 485 to circumvent the retrogression, based on my approved 140 and HSPA. Please advise me. Thank you.

If you are from India, You cannot. You can dream about doing it but unfortunately until your priority date is reached you are stuck. From the fact that you filed for PERM, you probably have a very recent priority date..........unless you converted an old labour case.
Hence my friend you like many others will be stuck holding you approved 140 for a long time.
