New in getting H4 visa - please help!!!!

I have H1B visa and want to take my kids here. Please advise me on what documents should I prepare for H4 processing. Is there an application/petition to be made to USCIS? Or, the kids just go to the US Consulate for interview with my H1B papers and DS 156? How much for each child will I spend for the H4 processing? What should I do???? :confused:
