Need some small help with I-751.

I am a nervous wreck with these things. I am sorry if this turns out to be a dumb question but I don't want to take any chances. On Part 4 "Information about the spouse" it asks for the spouse's "Family Name". Is that her last name right now (she took my last name) or her maiden name? Also, I assume that since she is now a citizen, she does not have an Alien Number any more, right? Any help is much appreciated.

Also, the following is a list of supporting materials I'm submitting, does this sound like enough?

Copies of joint tax returns (06,07,08)
Bank statements for joint checking account.
Bank statements for joint savings account.
Bank statements for joint CD account.
Copies of applications to open all the above accounts.
Copies of apartments leases with both of our names.
Copies of car insurance policies with both names.
Copy of mortgage pre-approval letter with both of our names.
Copy of doc showing my wife is enrolled in my job's health insurance policy.
Copy of doc showing my wife is enrolled in my job's dental insurance policy.
Copy of doc showing my wife as the beneficiary of my life insurance policy.
Itinerary for our trip to Paris last year.
Pictures of various trips we've taken.
2 affidavits from friends.
Copy of Marriage certificate.
I-797C notice of action telling my wife I have been approved for GC.

Is that enough, are there other things we should include? Thanks to everyone for their feedback!
 
Did she got SS & ID changed after marriage, meaning is it showing your last name, then mention your last name. check tax returns also, just for confirmity. you can leave A# box blank.

Thanks. She did change her SSN and ID to my last name. All joint accounts, joint tax returns, her ids and her naturalization certificate have her with my last name as we got married before she was a citizen.
