Need Help with I-765 Form

I have a question on one of the entries on Form I-796. Item 11, Have you ever before applied for EAD from USCIS?

I applied OPT after graduating from school. I used my OPT for about 1yr before changed to H-1B. So should I answer 'Yes' to this question? If yes, does anyone know what kind of document is required? I didn't keep a copy of the form I submitted to the International Student Office when I applied my OPT. Thank you

You would say YES and submit a copy of your old OPT card - no need to submit the old application.

You should first try to see if your old employer has a copy in their files.

max2K1 is correct. You have say "Yes" and submit a copy of the front and back of the card. I did the same thing.

Hello

I am in same boat as the OP. I do not have a copy of my EAD and I am very doubtful it my employer has a copy either (over 6 years ago). What else can we do? Do we have to submit documentation or only if you have it?

Not sure what to do?
