Need Help----Where can I get the fax inquire form?

My RD:5/30/2001, ND:08/06/2001. I am waiting for FP notioce. It is almost 4 moth after ND. I am going to send a fax inquire.

I have a CSC procedure in PDF format (about 1 MB) which contains the inquiry form.

Sorry, I do not have the original PDF file. However, I can scan the form and post it.

If anyone have the original file "CSC guideline" which I download from this forum, please post it again.
