Need help in filling the form AR-11

Hi Guys,

I'm working on EAD and my I-485 is pending. Recently i moved to a new address.
While filling out the 'AR-11' form, there are two fields which i'm not sure what to fill in for those two places.

1). I AM IN THE UNITED STATES AS:
options are visitor, permanent resident, student and Other....

2). IF NOT A PERMANENT RESIDENT, MY STAY IN THE U.S. EXPIRES ON:

I'm not sure what to fill in those two fields. Can somebody please let me know.

1. I-485 Pending ( AOS Pending )
2. NA or I-94 date if the date is a future date

Send USPS with delivery confirmation and keep the receipt !

I would suggest you send it as USPS Certified Mail with return receipt.

Guys, this is my question on this. I just dont want to make any mistakes while sending this form.

On the AR-11 form, it says 'copy number from alien card'.

Where can i see this number ? is it on I-485 notice of action ? Is it I-485 EAC number or Is it the same number which shows up 485 notice as A## number ?
