Need Help Filing AR-11

Gents,

Please help me filing AR-11. I have following questions.

1. Do we need to fill separate form for each family member, if yes can we post them all together in one envelope.?

2. What needs to be filled under "I AM IN UNITED STATES AS.."
(I am working on EAD , waiting for 485 approval)

3. Port Of Entry should be filled in as original (First) entry or the last entry into US. If it is supposed to be last entry, I entered from Canada and they did not change my I-94 card. What Should I fill for "Port of entry into US " and Date Of Entry into US

Yes for each family member.You can post in a single envelope. But my advise would be post in a separate envelope with a proof of despatch.

Ur last port of entry when ur I-94 was changed.
 
In addition to AR11 do not forget to call up BCIS and personally ensure that address change is done.Normally these AR11 requests take a lot of time (i wonder whether they really act on this) for address updation. So call up and get the address chnage done telephonically.
 
I called BCIS yesterday and updated my address. Looks like it was some kind of national center as the lady asked me about the center where my application is filed. She took all my information and told me that they will inform the center. I am not yet sure whether I called the right number or not?
 
Earlier we used to call the respective service center directly.Now they consolidated all of this at National level.I don't have the number but U can find this out in this forum.
 
A# ? on AR11

Do I need to fill A # while my I-485 still pending. As the A # is the number from Alien card? I have EAD card and no alien card. Can I leave the A# blank on AR11 form?

There is just one A#

that can be found on ead card or even the petition. even the AP will have the same A no.
