need for new I140 if NIW job change

Hi
Is there a need to file a new I 140 if there is a job change while doing the 5 year NIW physician waiver. I have been getting conflicting reports, some saying it is needed, some saying its not.

Any body has any links to relevant USCIS.gov sites.

after your three years of waiver if you need to change job you can file for an amended 140 and change jobs as long as that area is still MUA /HPSA and the DOH gives you a letter of support( which i think is important), as long as it is ok with DOH, INS would have no problem approving your amended 140
 
some lawyers say that if its a self petition then a new I 140 is not needed even if someone changes job to a new underserved area while doing NIW time, any truth in that?. i have heard this info from at least 2 different sources ( lawyers)
 
well then a someone should confirm or deny it, my lawyer Mr khanna specially told me that one has to file for amended 140 , if you hear any thing affirmative please let me know also, hadron or J1 victim any comments
