Need Advice on AP (I-131) Emergency

I applied for Emergency AP only ie I dint apply for regular AP or had pending AP. I took letter from surgeon about illness and some supporting documents (like prescription letter etc). I did get a letter from local Congressman office too. I had to fax them my documents and congressman office had setup an appointment for me.

Could you please advise what all documents you took with you
when you went to local office?

Did you submit a new I-131 form at local office? Was your
I-131 pending at the Service Center why you went to local office?

Please give an outline of what all things to take'
