I am filling N400 form. It has a question that says "Your name exactly as it appears on your Permanent Resident Card." There it asks for "Full Middle Name" (if applicable). On my GC I have my name as Roy Seema M. So it has middle initial of M. So should I write my middle name as M or spell it out completely "Mayur". I am asking since on one had they say name exaclty as in your PR card - but then for middle name they say full middle name. On the PR card I only have middle initial - so what should I write the middle initial OR the full middle name?

Thanks a lot for your help. - Seema

My wife has same situation...Her drivers license has full middle name so she put full middle name in the first part. In the second part ( name as on greencard) she put M as middle name as thats the way it is on GC.

I was in a similar situation before I sent the application a few days ago. The new GC always has your middle initial on the top of the card where it says "name" but if you look to the bottom then you will see that it spells out your whole name.

I put my name with the middle initial A and then wrote out the whole name as it is on my birth certificate where is says "legal name."

Hope this helps.
