my wife is going to endorse my name in her passport. when she goes to take her H4 visa at that time In DS 156 form what should she write as her name? her original maiden name or her name+ my surname?Also when you endorse the name in your passport , do they change the first page in the passport or they just add the husband's name in the last page of passport?
