Name Differences in passport & H1B approval notice - Urgent!!!

Hello Experts,

It would be great help if someone can help me this isse. My wife is currently on valid H1B visa, but there is name difference in passport & H1B approval. She is planning to travel to India in October for H1B visa stamping. Below is the brief description:

1)Name in Passport: Surname: PARVEZ SADIYA PARVEEN
Given name: SYED ZUBAIRUL HASAN ( which is my name, added to her passport after marriage)

2) Name in H1B approval notice: HASAN, SADIYA P.

Since the name in H1B approval is HASAN, SADIYA P. her Social Securiy card, paystubs & other documents are also having the same name. Now She is planning to travel to India in October for her H1B visa stamping.

3) Will this difference in name in passport & H1B visa will create any problem for her visa stamping.

4) If this will create a problem, what is the possible solution? Please suggest??

5) Yesterday I submitted fees for her H1B visa stamping appointment in India. They took the visa fees as per her name in Passport ( PARVEZ SADIYA PARVEEN ). Now I have to take her appointment & fill all the forms. Should I fill the forms as per her name in passport or per her name in H1B visa approval & other documents( paystub, SSN, employment letter etc.).

Any help & suggestion is very much appreciated. Its very important for her to travel to India in October.
