My wife filed for the I-90, and so we have a date for the ASC Appointment, and currently her Green Card has her maiden name on it, and we want to change to her married last name. We were married outside of the country, so all we have is a copy of the marriage certificate (from Costa Rica), so it's in Spanish, can we use this form for the name change? It a yellow copy, so like a carbon copy of the original.
