All,

My wife has put in a request for a name change in her N-400. We have yet to receive any IL etc.

In the event that it is granted and her name change in official, what additional document does one need to send along with the OCI application form to show that the person in the old Indian Passport and new US passport is the same person?

Thank you

My case is the same, and I did not send any additional information. Copies of the Naturalization Certificate and the Passport copies should suffice...

ShriniB,

In your case, is it only the last name that is changed?

My wife is actually shortening her name, eg. Jaswinder to Jassie.

What are implications on the Children's OCIs? Their BC have her previous name.

I would think a self-declaration (notarized, if possible) describing the name change (reason, circumstances, date, etc.) should take care of this.

While in our case it was a last name change, my take is it should not matter. Name change is name change (first, middle or last name).

In my opinion you will not need any additional docs... naturalization certificate (showing your wife's current name) and her Indian passport (showing the previous name) should be sufficient.

Good luck with your case...

Thank you akgarg and SriniB for your input. I think between the US passport and Indian Passport, I will include a notorized self-declaration document to link the two. That should be it.

My wife changed her last name to my name when she got her US Citizenship. We applied PIO card for her since she needs to go to India immediately (otherwise we would have applied OCI). It is been almost 4 weeks and We just got a voicemail from Houston consulate saying they need a proof regarding name change. The only proof we have is her new US passport with the new name and her old Indian passport with the old name. We sent both with the PIO application. I am going to send a notarized self declaration and see.

Do you have any other suggestion? What else can i do? We need to get the PIO card immediately as she needs to travel to India soon. I tried calling Houston phone number several times without any success.

When you get your citizenship certificate, your wife will get an **additional name change certificate** also. When you apply for OCI, just send a copy her INDIAN passport with her old name, US passport with the new name and the COPY OF HER NAME CHANGE CERTIFICATE (notarized). I just did the same thing...

I applied for OCI after my first name change. Here's what I did and OCI is granted.

Copies of Indian Passport and US passport (as described in consulate's website)

**Since I changed my first name, i put self declaration page (full name and signed at the bottom), a copy of driver's license (reflecting new name), and a copy of a name change certificate (obtained from the district court during the Oath ceremony). **

**Include same thing for your other pile.
